Statement of Purpose

This survey is designed to identify the personal needs and concerns of colorectal cancer survivors in the Lubbock and surrounding counties. Lubbock Colon Cancer Prevention Task Force with its partners will conduct the pilot survey using grant funds from the Texas Cancer Council. The data collected will be used to find out how to reduce problems faced by colorectal cancer survivors and how to increase access to cancer services.

This survey has been developed by the Cancer Care Services, a nonprofit organization in the Forth Worth/Dallas Area, with funding from the Texas Cancer Council grant. Lubbock Colon Cancer Prevention Task Force is a nonprofit group, operating under umbrella of American Cancer Society, with mission to eliminate illness and death from colorectal cancer in our region. The Texas Cancer Council is a state agency that administers implementation of the Texas Cancer Plan with emphasis on cancer prevention, early detection and treatment, professional education and practice, cancer data acquisition and utilization, and improving quality of survivorship.

Participation in this survey is voluntary. Colorectal cancer survivor volunteering to complete the survey will sign a permission sheet indicating their willingness to participate in the survey. Permission
Sheet will be held in the Lubbock Colon Cancer Prevention Task Force office. Survey questionnaire does not have place for name of person completing it, thus it will be anonymous. Data from this pilot survey will be analyzed and reported to Texas Cancer Council. This data may be used for planning and educational purposes.

Survey Instrument Design

The survey instrument was designed to identify social service needs of patients and to study the relationships between quality of life and patient needs. Each instrument contains four question domains: (a) 7-items related to general health/treatment; (b) a 40-item ordinal scale that measures quality of life; (c) 37-items designed to identify needs; and (d) a 14-item demographic profile. Based on preliminary timing tests, the estimated completion time for patients is 20 minutes.

The items on the survey were originally developed from a review of existing cancer patient research from various sources, including City of Hope National Medical Center, the World Health Organization, and several studies reported in Quality of Life Research journal. The quality of life scale is reproduced without modification as designed and tested by the City of Hope (National Medical Center and Bechman Research Institute). The City of Hope has tested this scale for reliability and validity for four domains of quality of life, including physical well being, psychological well being, social well being and spiritual well being. Specific methodology and test results can be obtained at http://www.cityofhope.org. The City of Hope grants permission to use and duplicate this scale and a companion scale for caregivers on their website.
